Evaluation includes formative evaluation and summartive evaluation. Formative evaluation is present in each phase of the ADDIE process. Summartive evaluation includes Kirkpatrick's four-level evaluation:

• Reaction - how the learners react to the learning process (usually use survey to get the information about learners' satisfication)
• Learning - the extent to which the learners gain knowledge and skills (usually use criterion-referenced test to test learners' knowledge and skills)
• Behavior - what was learned is being applied on the job (usually use observation or interivew method to get the information)
• Results - measure whether the training is achieving results

I did one project about evaluation of the Blackboard services provided by Instructional Support Services in my university. We did this project in a group of three persons. We interviewed 10 faculty members and 20 students who are teaching or learning by Blackboard. We also interviewed 3 staff members in Instructional Support Services. We did quality evaluation using interview and observation methods. Sorry, I cannot upload my report here because we have promised that we won't public the report. But I did learn a lot evaluation techniques from this project.
